Public Health Nurse 1 or 2 / home health &
Hospice
The homecare division is seeking a registered
nurse for the position of Public Health Nurse/ Home Health &
Hospice.
Responsibilities include:
- The delivery of services to home
health/ hospice program participants; interviews and assess
patients to determine needs for services, develop and administer
medical plans based on patient needs, works with all disciplines
and the patient to establish a comprehensive plan of care.
- Preforms comprehensive assessments, provides
skilled interventions aimed at achieving outcomes and goals with
in specified time periods; modifies, evaluates and updates plan
of care to reflect patient needs and outcomes; consistency
demonstrates competency with technical nursing skills.
- Conducts home visits to provide routine and
specialized direct nursing care.
- Prepares required records and accurately
maintains records for submission in a timely manner.
- Provides for the safety of the patient.
- Provides for the education appropriate to the
patient’s/family needs
- Provides guidance to Home Health Aides.
- Demonstrates regular and predictable
attendance; must be able to work assigned evenings, weekends,
and holidays.
Must maintain State of Ohio driver’s license, must
maintain insurability according to policy.

Public Health Aide 1 / Home health & Hospice
The homecare division is seeking a
registered nurse for the position of Public Health Aide/ Home Health
& Hospice.
Responsibilities
include:
- Provides routine personal care for patients
in the home as assigned; under the direction of Home Health
Nurse
- Preforms simple dressing changes, monitors
records vital signs, observes patient status, documents
observations
- Performs light housekeeping duties
- Prepares
charts and patient records as assigned
- Provides in-home personal hygiene assistance
to clients
- Demonstrates regular and predictable
attendance; must be able to work assigned evenings, weekends,
and holidays.
- Must maintain State of Ohio driver’s license,
must maintain insurability according to policy
Employment Status: Intermittent

Intermittent Dental Assistant I
Responsibilities include:

Prepares operatory for patient dental treatment (e.g. Makes sure
operatory is clean, sterilizes instruments and stores
instruments in sterilizing bags)

Gives direction to patients as authorized by Dentist or
Hygienist
-
Assist dentist in general dentistry procedures
-
Takes and develops x-rays as required
-
Answers patient questions and concerns regarding procedures in a
professional and empathetic manner
-
Maintains inventory of dental supplies
-
Ensures proper disposal of all contaminated or potentially
contaminated materials
-
Able to travel as necessary to meet clinic’s operational needs
-
Demonstrates regular and predictable attendance, has reliable
transportation
-
Meets all job safety requirements and all applicable OSHA safety
standards that pertain to essential functions
-
Maintains all required licensure and certifications, including
continuing education units (CEU’s)
-
Participates in HD Disaster Preparedness activities
-
Experience Preferred
-
Will be scheduled as needed for vacation coverage and to cover
call-offs
