Henry County
Family Planning receives funding from the Ohio Department of Health,
Reproductive Health and Wellness Program Title X grant.

Henry County Family Planning provides quality reproductive health
services for both men and women. Clinics are held four times per
month, twice at the Henry County Health Department and twice at our
satellite clinic in Defiance County.
All services are strictly confidential and provided by
licensed medical professionals including Physicians, Nurse
Practitioners, Registered Nurses and a caring support staff.
Services Include:

Physical examinations including pelvic exam, pap
smear, and clinical breast exams
·
STD testing and treatment for males and females
·
Pregnancy testing, counseling, and referral
·
Level 1 infertility testing
·
IUD insertion and removal
·
HIV testing
·
Laboratory testing as ordered by clinician
·
Abstinence Education
·
Birth control including oral contraceptives,
depo provera injection, diaphragms, natural
family planning instruction, condoms, IUDs, vaginal ring and emergency
contraception.
Fees for
services are based on your household income. Please bring income verification to your appointment (last two pay
stubs, last years tax return or an employee statement). Henry County
Family Planning is able to offer some services for free or at a
reduced fee based on where you fall on our sliding fee scale.
Application. While you may
receive services at no charge, we encourage donations at each visit
to help cover the cost of the services you receive.
Medicaid and some private insurance is accepted. Please bring your
insurance card with you to each visit.
No one is denied
service due to an inability to pay. |
