Who are these
clinics for?
These clinics
are for infants through adults who need immunizations.
What should you
bring?
All children
under 18 years old must be accompanied by a parent or guardian.
If a parent or guardian is unable to bring the child themselves,
another trusted adult that knows the child’s history and is able to
answer general health questions about the child may bring the child
as long as that person brings along a signed note from the parent or
guardian. This note must state that this person has permission to
sign for the child to receive vaccinations. A new note must be
brought for each vaccination visit.
A copy of past
immunizations (immunization record) must also be brought to the
immunization clinics. If the child has
a medical card, that should be brought along as well.
What does it
cost?
A donation of $ 9.00 is
requested for required vaccinations, but no one
is turned away due to inability to pay for required vaccinations.
The cost of recommended vaccines varies depending on
the vaccine to be given.
Please contact
the Health Department for specific vaccine recommendations or
requirements.
