Who are these clinics for?

These clinics are for infants through adults who need immunizations. 

What should you bring?

All children under 18 years old must be accompanied by a parent, guardian or authorized caregiver*.  Bring a copy of past immunizations (immunization record), and a medical assistance card, (If the child has one) to the immunization clinics.
*NOTE: If some other trusted individual other than the parent or guardian is bringing the child for immunizations, a
 third party immunization permission form MUST be completed by the parent or guardian and brought with the child.  This form MUST be completed for each immunization visit. 
The screening questionnaire for Child and Teen Immunization may also be completed by the parent and brought along with the child, as well as the
Henry County Health Department Consent/Authorization for Immunizations and Release of Information form.

What does it cost?

A donation of $ 12.00 is requested for childhood vaccinations, but no one is turned away due to inability to pay for required vaccinations.  The cost of recommended vaccines varies depending on the vaccine to be given. Adult & travelers vaccine costs vary depending on the vaccine.
