Birth and Death Certificates
The Henry County Health
Department can now provide certified birth records (certificates)
for
anyone born in the state of Ohio.
Certified death certificates
are available for any person who died in Henry County beginning in 1909.
We cannot access death certificates if the death occurred in
another county at this time.
A written request must be received that
includes the person’s name on record, date of birth or death, and an
address where to send the certificate.

Cost: $23.00 for each certified copy requested.
DEBIT CARDS are now being accepted as payment when
ordering a birth or death certificate in person at the Health
Department. A $2.50 fee is charged for using this service.
INTERNET ORDERS
Remote applications
for Birth and Death Certificates are available through
VitalChek , a
website application. Payment of $23.00
for each certificate ordered is made by credit card.
A
service fee is charged for each order. Records can be mailed
first class with no charge or shipped by UPS by paying shipping charges.
