Birth and Death Certificates
The Henry County Health
Department can now provide certified birth records (certificates)
anyone born in the state of Ohio.
Certified death certificates
are available for any person who died in Henry County beginning in 1909.
We cannot access death certificates if the death occurred in
another county at this time.
A written request must be received that
includes the person’s name on record, date of birth or death, and an
address where to send the certificate.

Cost: $23.00 for each certified copy requested.
DEBIT CARDS are now being accepted as payment when
ordering a birth or death certificate in person at the Health
Department. A $2.50 fee is charged for using this service.

An Electronic Index is
now available for birth and death records, located in our waiting area,
during normal business hours of 8:30 am to 4:30 pm, Monday - Friday.
Clients may log on to the electronic index to search for birth records
dating from 1909-present and death records dating from 1954-present for
the entire State of Ohio. Death records from 1909-1953 (Henry County only)
can still be searched using the genealogy book located on the electronic
